Here's what you need to fill out under the powers/abilities field in your character journal.
Power Name:
Power Description:
That's all you need. In the power description, go as in depth as possible, the more detail the better. Name what said power does, what's good about it, weaknesses, how it's activated, how powerful it is depending on your character, and how much the power can effect another player/how much it can do.
Here's an example of a power sheet I filled out for another site:
See how I went all the way as to describing what's wrong with it and how the power is so strong anything goes? There will always be more flaws than virtues, and I'm sure a proxy won't have their power mastered, so try and avoid OPness. You should never make your power/ability perfect, and it won't always be effective or beat another's.
It isn't just powers that can go here. Abilities, such as fighting (e.x. kung-fu ), could go here too, and you could explain how skilled your character is in the area.

Sealing
"This ability is special to Felicity, seeing as it was earned in Hell, and branded specially into her skin for herself. The symbols on her chest and stomach can hold items of Felicity's choice. However, the chest area can only hold one item at a time, mainly being her sword. The stomach symbol can hold up to three items. There's a catch, however, to this ability. The items must have sentimental value to Felicity or she cannot place them into her body. The items can be up to 10 feet long, and 12 inches thick, anything surpassing that and the sealing symbols with deflect the object by causing sparks to fly and tossing the items out of Felicity's hand. Also, when pulling out an object from either of her symbols she must say,"Uchi ni oide" or "Come to me" in Japanese. When placing something back into her body she must say, "Kitaku" or "Return home" in Japanese. If someone were to slide the sword into her chest, or try to, it wouldn't work since Felicity must to do it and say the key words. This is required for anything being pulled from the body. Another catch is that the object being retrieved from the symbol or placed back in must be pulled out or shoved in slowly or it may harm Felicity, such as cutting her skin deeply, but nothing more than that. It's still a risk, and Felicity prefers to pull out the items slowly. Currently she's only holding her sword in her chest symbol."
